Privacy Policy

This policy, dedicated to the collection and management of personal information collected via Clinique IUVO’s website or by any other technological means (hereinafter referred to as the “Policy”), has been formulated in compliance with the provisions of the Act Respecting the Protection of Personal Information in the Private Sector (RLRQ, c. P-39.1). The Policy describes how we handle and protect the personal information we collect through technological means (e.g. e-mail). The main purpose of this Policy is to inform you of the measures taken by the Clinique to protect your personal information.

 

Consent:

By using our website or by providing us with personal information (for example, by e-mail or by registering on our waiting list), you consent to the collection, use and disclosure of your personal information for the purposes described below.

 

You may withdraw your consent to the use and disclosure of your personal information at any time by contacting us at info@cliniqueiuvo.ca. Withdrawing your consent may prevent us from providing or continuing to provide you with some or all of our services. We may be required to retain certain personal information to comply with our legal obligations.

 

Collection of Personal Information:

 

Clinique IUVO only collects your personal information for the following purposes:

  • provision of speech-language pathology and neuropsychology/psychology services;

  • administrative management of our activities;

  • compliance with legal and regulatory requirements;

  • responding to requests for information or services;

  • any other legitimate purpose.

 Personal information is collected through our website, e-mails and cookies.

 

Personal Information Collected:

 

The information we collect varies depending on the services requested. Information may include, but is not limited to, the following:

  • identifying information such as last name, first name, date of birth, address, telephone number and e-mail address;

  • medical information, as well as family and school history (kept only in speech therapy and neuropsychological/psychological records);

  • information relating to speech therapy evaluation and treatment, neuropsychological evaluation and psychological treatment (stored only in speech therapy and neuropsychological/psychological records).

  • browsing information (IP address, pages visited, connection time)

 

Use and Disclosure of Personal Information:

Clinique IUVO uses personal information collected only with your consent and for the purposes for which it was collected, such as the provision of services and administrative management. This information is not shared with third parties without your explicit consent, except where the law allows us to do so (e.g. following a communication request from our professional order, to comply with a court requirement).

 

Retention of Personal Information:

Personal information is stored securely and in accordance with professional standards. It is retained for as long as necessary to fulfill the purposes for which it was collected, unless otherwise required by law. Information contained in a client's file is retained for five years after the file is closed, and then destroyed in accordance with the law.

Cloud-hosted information is hosted in Quebec. If it is communicated or hosted elsewhere, Clinique IUVO will ensure that it is adequately protected and that an appropriate agreement is in place.

 

 Security of Personal Information :

Appropriate security measures are in place to protect personal information against unauthorized access, loss, disclosure, alteration or destruction. This includes technical, administrative and physical measures such as restricting access to information, the use of security protocols, staff training and regular assessments of security practices.

 

Access and Correction of Personal Information:

Clients have the right to access their personal data held by Clinique IUVO and to request the correction of any inaccurate or incomplete information. Requests for access or correction should be addressed in writing to info@cliniqueiuvo.ca. Clinique IUVO will respond to such requests within the time limits provided by law.

It is your responsibility to notify us of any changes to your personal information, so that we can keep it up to date at all times.

You may also request that we cease disseminating your personal information (de-indexing) or submit a portability request (as of September 22, 2024) by sending us a written request to info@cliniqueiuvo.ca.

Making a Complaint:

If you wish to file a complaint regarding this Policy or its application, you may do so by contacting the person responsible for the protection of personal information at info@cliniqueiuvo.ca.

Changes to the Policy:

Clinique IUVO reserves the right to modify this Policy to reflect legislative developments or good privacy practices. You will be notified of any material changes to the Policy by reasonable means, such as publication on our website or by e-mail.
